Kindly note that Dr. Wong’s current practice will be closing in March 2025. They will continue to provide follow-up appointments for all current patients until this date.
An email was sent to all existing patients on October 21st, 2024 from Dr. Wong with further details.
As mentioned in the email, patients will be notified if Dr. Wong is able to relocate to another office before the above date. However, this is not guaranteed as moving a medical practice is complex, so we strongly recommend that you contact other clinic(s) to transfer your care over to ensure you have continuity of care.

Can I see dr. Wong again?
Dr. Wong will typically see patients 8 to 12 times in follow-up. They will provide instructions for when they would next like to see you, and whether in-person or by video.
If you haven’t seen Dr. Wong in 2 years or more, Ministry of Health regulations require a new referral.
Please have a referring physician (e.g. your family doctor) fax a referral to 416.572.7103.
How do I reschedule or cancel my Follow-up appointment?
If you consented to receiving an email confirmation, you should have been sent an email confirmation with a link to cancel or reschedule. For most individuals, your credentials to cancel or reschedule your appointment:
Username: your email address
Temporary password: your OHIP number.
If you already happen to have an account with Veribook, our online scheduling system, you will need to use your existing account password instead.

Please show consideration to your fellow patients by cancelling appointments as early as possible if you no longer require an appointment or can no longer attend. Kindly note that all missed appointments, or those cancelled without sufficient notice will be subject to a fee, payable before another appointment may be scheduled.
Sufficient notice is 72 hours for follow-up appointments, and 7 days for subsequent intake appointments (i.e. hormone therapy / surgery readiness assessments).
It is also an option to call our booking service between 11 AM and 1 PM, Monday to Thursday, at 647.812.2089 ext. 225 to schedule appointments. Please note that the booking agents will not be able to help you with medical or administrative questions outside of questions related to booking an appointment.
HOW DO I PREPARE FOR and attend MY Virtual Follow-Up APPOINTMENT?
1. You must be a resident of Ontario AND physically be in Ontario at the time of the appointment.
2. Access Dr. Wong’s virtual room at least 5 minutes before your scheduled appointment time.
Unfortunately, to help your doctor stay on schedule, if you arrive late for your appointment, there is a possibility that you will not be seen, or that you will be asked to wait until the next break in the schedule.
3. Cancellation: Please show consideration to your fellow patients by cancelling or rescheduling appointments as early as possible. A fee will be payable in the event of a missed appointment, or appointments cancelled less than 72 hours in advance. Information about how to reschedule or cancel can be found on this page under “How do I reschedule or cancel my follow-up appointment?”.
4. Please note that your appointment may not start right on time. Please be patient with us and know that your flexibility is vital in keeping our community safe.
5. Privacy: Arrange to have your virtual appointment in a private and quiet room where you will not be interrupted by other people or disturbed by noise. Please use a private computer, phone, or tablet, with a secure internet connection and camera.
HOW DO I PREPARE FOR and attend MY In-Person Follow-Up APPOINTMENT?
1. Cancellation: Please show consideration to your fellow patients by cancelling or rescheduling appointments as early as possible. A fee will be payable in the event of a missed appointment, or appointments cancelled with insufficient notice. Information about sufficient notice, and how to reschedule / cancel can be found on this page under the section titled “How do I reschedule or cancel my follow-up appointment?”.
2. Appointment Location: your appointment will be at 418 Eglinton Ave. WEST, 2nd Floor, Toronto, ON.
3. Plan for your appointment to take up to 1 hour.
4. Arriving: Please arrive 15 minutes early for your appointment to allow time for screening and any other appointment prep required by the physician. Please keep in mind there is currently an increase in traffic congestion due to construction. Kindly take this into account when planning your travel to the clinic.
5. Late Arrivals: To help your doctor stay on schedule, if you arrive late for your appointment, there is a possibility that you will not be seen, or that you will be asked to wait until the next break in the schedule.
6. Self-Screening for In-Person Appointments:
Within the past 5 days have you or any individuals in your group had any of the following symptoms?
Fever and/or chills, cough, shortness of breath, sore throat, headache, decrease in sense of smell or taste, runny nose, or nasal congestion?
If you do not have any of the symptoms listed: You have screened NEGATIVE. You may continue with your in-person appointment, and nothing further is needed from you.
If you do have any of the symptoms listed: You have screened POSITIVE. You may continue with your in-person appointment but kindly:
bring a mask to wear within the clinic at all times, including when you are in the exam room
complete a Rapid Antigen COVID-19 test prior to your appointment if possible (find a participating pharmacy/grocery location here). Having the results from a COVID-19 test is helpful to ensure appropriate precautions are taken to keep the community, staff, and physicians safe.
