IMPORTANT NOTICE:

Please note that Dr. Monsour will be relocating his practice to a new office in Oakville in February 2025. He will continue to see patients out of his current office space (418 Eglinton Ave W) through January 2025.

The new clinic address is: 1943 Ironoak Way, B101, Oakville, ON, L6H 3V8

An email notice was sent to all current patients on August 1st, kindly refer to that email for further information.

Kindly follow up with your primary care provider should you wish to be referred to an alternative specialist.


Can I see dr. Monsour again?

  • Only if you have seen Dr. Monsour about this specific concern before

  • That last appointment was within the last 2 years

If the above does not apply to you, Ministry of Health regulations require a new referral. Please have a referring physician (e.g. your family doctor) fax a referral to 647.480.5031.

Alternatively, to book your appointment you can call our booking office between 11 AM and 1 PM, Monday to Thursday, at 647.483.0690 ext. 225 to schedule appointments. Please note that the booking agents will not be able to help you with medical or administrative questions outside of questions related to booking an appointment.

Note: all available appointments are shown online. The booking agents do not have the ability to make exceptions.

How do I reschedule or cancel my appointment?

If you consented to receiving an email confirmation, you should have been sent an email confirmation with a link to cancel or reschedule. For most individuals, your credentials to cancel or reschedule your appointment:

Username: your email address
Temporary password: your OHIP number

If you already have an account with Veribook, our online scheduling system, you will need to use your existing account password instead.

You can also sign into your account directly here, if you lost the email confirmation.

Please show consideration to your fellow patients by cancelling appointments as early as possible if you no longer require an appointment or can no longer attend. Kindly note that missed appointments, or those cancelled without sufficient notice will be subject to a fee, payable before another appointment may be scheduled. Sufficient notice is: 48 hours for virtual appointments and 72 hours for in-person appointments.


HOW DO I PREPARE FOR and attend MY Follow-Up APPOINTMENT?

VIRTUAL FOLLOW-UPS

1) If you were previously a patient of Dr. Monsour at Mackenzie Health and this is your FIRST appointment at his new practice, fill out and submit an Intake Form at least 7 days before your appointment. If this form is not filled out and submitted before your appointment we may have to reschedule your appointment.

2) If you are a patient who experiences migraines
fill out this form, at least 2 business days before your follow-up appointment.

3) If Dr. Monsour had instructed you to fill out a
headache diary, submit it here, at least 2 business days before your follow-up appointment.

4) Late Arrivals:
To help your doctor stay on schedule, if you arrive late for your appointment, there is a possibility that you will not be seen, or that you will be asked to wait until the next break in the schedule.

5) Cancellation: Please show consideration to your fellow patients by cancelling or rescheduling appointments as early as possible. A fee will be payable in the event of a missed appointment, or appointments cancelled less than 48 hours in advance. Information about how to reschedule or cancel can be found on this page under “How do I reschedule or cancel my appointment?”.

6) Privacy: Arrange to have your virtual appointment in a private and quiet room where you will not be interrupted by other people or disturbed by noise. Please use a private computer, phone, or tablet, with a secure internet connection and camera.  

7) Kindly note all virtual follow-up appointments will be VIDEO only (i.e. it will not be an option to attend virtual follow-up appointments via phone). We appreciate your understanding and cooperation in this matter.

Access Dr. Monsour’s Zoom virtual room at least 5 minutes before your scheduled appointment time.
Dr. Monsour will bring you into the virtual room when done with the patient before you.

Please note that Dr. Monsour may be running up to 1 hour behind due to complexity of care.
Kindly remain in the waiting room, and Dr. Monsour will let you in as soon as he can. Rest assured your appointment will still happen, we do not cancel any appointments without notifying you.

Make sure to use your name - otherwise you will not be admitted to the room.


HOW DO I PREPARE FOR and attend MY Follow-Up APPOINTMENT?

In-Person Follow-UPs & Procedures

1) If you are a patient who experiences migraines fill out this form, at least 2 business days before your follow-up appointment.

2) If Dr. Monsour had instructed you to fill out a headache diary, submit it here, at least 2 business days before your follow-up appointment.

3) Appointment Location: Please ensure that you check your appointment confirmation email for the address of where you will be seen. Dr. Monsour sees patients at more than one location.

4) Plan for your appointment to take up to 60 min.

5) For All Procedures: Avoid wearing a white shirt, or a shirt that covers your neck & if you have long hair, bring a hair-tie/elastic to tie your hair up.

6) Nerve Blocks: Arrange for someone to pick you up from your appointment as you may feel lightheaded afterwards.

7) Botox Procedures: If you are having a Botox Procedure that requires payment, we accept credit card or exact cash (no change available).

8) Arriving: Please arrive 15 minutes early for your appointment to allow time for screening and any other appointment prep required by the physician. Please keep in mind there is currently an increase in traffic congestion due to construction. Kindly take this into account when planning your travel to the clinic.

9) Late Arrivals: To help your doctor stay on schedule, if you arrive late for your appointment, there is a possibility that you will not be seen, or that you will be asked to wait until the next break in the schedule.

10) Do not have any other Nerve Block, head, or neck, procedures within a week of the appointment.

11) Cancellation: Please show consideration to your fellow patients by cancelling or rescheduling appointments as early as possible. A fee will be payable in the event of a missed appointment, or appointments cancelled less than 72 hours in advance. 12) Self-Screening for In-Person Appointments:

Within the past 5 days have you or any individuals in your group had any of the following symptoms?

  • Fever and/or chills, cough, shortness of breath, sore throat, headache, decrease in sense of smell or taste, runny nose, or nasal congestion?

If you do not have any of the symptoms listed:
You have screened NEGATIVE. You may continue with your in-person appointment, and nothing further is needed from you.

If you do have any of the symptoms listed:
You have screened POSITIVE. You may continue with your in-person appointment but kindly:

  • bring a mask to wear within the clinic at all times, including when you are in the exam room

  • complete a Rapid Antigen COVID-19 test prior to your appointment if possible (find a participating pharmacy/grocery location here). Having the results from a COVID-19 test is helpful to ensure appropriate precautions are taken to keep the community, staff, and physicians safe.
